Make a Referral
We accept patients privately or via Care Commissioning Group referrals (CCG).
Upon receipt of a referral, the Bagshot Park team will:
Undertake a detailed pre-admission assessment
Encourage prospective patients and their loved ones to visit our centre and meet our team
Prepare a detailed, person-centered report of care needs, including risk assessments and proposed fees
We hope to make the process from referral to admission as smooth as possible. All assessments, where possible, will be carried out in the current residential setting of the patient. We will liaise with the current care provider to obtain as much information as possible regarding their clinical, care and rehabilitation needs.
** If it becomes clear from referral or assessment that we will be unable to meet the needs of any patient we will signpost the referrer to another service or provider wherever possible.
